Althoa is the second largest of the Telvanni Isles, after Vahn Isle in the main archipelago. Although located north of Firewatch just off the coast of the Sunad Mora region, the island is still considered part of the Telvanni Isles region.
The island is elongated in form, stretching from a narrow point in the southeast, widening out in the center, before narrowing slightly in the northwest. In the center of the island is the small fishing village of Bahrammu, while the Imperial colony of Nivalis crowns the island's northernmost point. The Telvanni tower of Tel Rivus occupies the southeastern section of the isle, sprouting from an ancient Dunmer stronghold.

- Althoa previously had the name of Balvvarden Isle, but this was changed in the Embers of Empire release, as 'vvarden' was considered a Dwemer word despite Balvvarden having no obvious Dwemer presence or history.
