This template is designed for use at the top of overview pages for unofficial mods, to provide a summary of the mod features and availability.

| title | optional | Title to use instead of "(mod name)" |
| image | optional | File name of the promotional art or in-game display |
| author | optional | Name of the mod author or author team |
| released | optional | Date on which the mod was first released |
| updated | optional | Date on which the mod was last updated |
| size | optional | Size of download |
| version | optional | Most recent released version of the mod |
| filename | optional | Filename(s) of the esm/esp/esl file |
| pc pcle pcse xbox ps4 |
optional | Summary of availability for the appropriate platform including link to download page (e.g. Nexus, Steam Workshop, Bethesda.net). Use pcle and pcse for Skyrim. |
| other | optional | Other ways to obtain mod. |
| patch | optional | Required patch level and/or any required official DLCs or add-ons. |
